As a Registered Social Worker I am excempt from charging GST and all fees are deductible as a medical expense on your income tax. Receipts are issued for reimbursement through your Extended Health Benefits Plan or for income tax purposes. The Employee Assistance Program through your workplace may also be of assistance.

CONSENT TO CLINICAL COUNSELLING AND USE OF PERSONAL INFORMATION

Experience and Orientation:
I am a Registered Clinical Counsellor and Registered Social Worker with more than 25 years of experience in the field of mental health as a social worker, counsellor, and psychotherapist. I have a Master of Arts Degree in Counselling Psychology and Dance/Movement Therapy from Antioch New England Graduate School in New Hampshire, USA.

Counselling Services:
Counselling provides a space and opportunity for you to explore behaviour, relationships, feelings, or thoughts, which trouble you and cause difficulty in your life. Counselling is also a source of support in a crisis or during a difficult time. Counselling can bring deeper personal insight and awareness, relief from troubling symptoms, better ways of understanding and coping with problems, and improved relationships. You should know, however, that counselling sometimes requires that you be willing to examine difficult topics or times in your life, to experience stronger than usual emotions and sensations, and to try out new and different behaviours.

Confidentiality and its exceptions:
Confidentiality is a key to the effectiveness of the counselling process, so the personal information you share in counselling will be kept confidential. Confidentiality continues after the end of the counselling relationship. There are, however, some exceptions to the counsellor’s duty of confidentiality, in particular:
(a) if a child or elder is or may be at risk of abuse or neglect, or in need of protection;
(b) if a counsellor believes that you or another person is at clear risk of imminent harm;
(c) for the purpose of complying with a legal order such as a subpoena, or if the disclosure is otherwise required or authorized by law.
(d) The counsellor may also disclose information for the purpose of a professional consultation, in which case your identity will remain confidential.

Confidentiality considerations with technology:
(a) Email and text forms of communication are not necessarily secure, therefore if you agree to communicate by email or text you waive your right to confidentiality.
(b) Video counselling – although I will make every effort to ensure security of our video counselling sessions, it is not possible to guarantee this. Thus you agree to accept the potential limitations of confidential counselling through video. Recording of sessions is not permitted by either therapist or client.

Fee and Payment Policy:
Fees: Payment by bank e-transfer is due before or at the end of each session unless other arrangements have been made.

Payment by a Third Party: Should the third party not pay the full costs of the clinical services that have been provided by the counsellor, the client will be responsible for those additional costs.

Receipts: I will provide a receipt acknowledging payment. As an RSW, my fees are deductible on your income tax as a health benefit and I am not required to charge GST.

Health Plan Reimbursement: The client is responsible for confirming the scope of coverage, and for seeking reimbursement for the cost of clinical services from any insurance, pension, benefit, or similar plan the client is a beneficiary of.

Cancellations: The time of your scheduled appointment is reserved for you. I require 48 hours notice for cancellations. It is my policy to charge in full for missed appointments or cancellations with less than 48 hours notice.  If you must cancel, please let me know as soon as you can along with instructions of when you would like to reschedule.

Concerns:
If you have a concern about any aspect of your counselling, you are requested to first address it with JENNIFER SCOTT. If this is impossible or unsafe, or if your concern is not resolved through discussion, you may contact the Registrar of the B.C. Association of Clinical Counsellors at 1-800-909-6303.
